How to Report

The Office of Investigations and Assessment can receive reports in multiple ways. Most commonly reports are submitted through report.fsu.edu using the “Report Student Organization Violations” or “Report Hazing” buttons. Reports may also be received from law enforcement, through email/phone, or in person in our office.

Florida State University's ability to investigate reported incidents, enforce the university’s expectations, and protect future students depends on the accuracy and specificity of the information provided. You are encouraged to provide as much specific detail as possible so that appropriate action can be taken to address the reported behavior.

I&A is an administrative office which is open during normal university business hours. In an emergency, individuals should contact emergency responders by calling 911 or the FSU Police Department at (850) 644-1234.

You have the option to submit a report anonymously by not including your name and/or contact information while filing a report online at report.fsu.edu; this is the only avenue for ensuring anonymity. However, please be aware that the University may be unable to complete an investigation and/or hold students/organizations accountable without the ability to share and evaluate the source of the report for credibility.
