Accommodation Application Process

The accommodation process is detailed below. Students may apply for disability-related accommodations at any time; however, any accommodations approved during the last ten (10) class days of a semester will not be active until the following term. Accommodations cannot be applied retroactively and are in effect ONLY after the instructor receives an Accommodation Letter. Each use of an accommodation must be communicated in advance and arranged according to the responsibilities associated with each accommodation. 

1. Be accepted to attend FSU and have a valid FSUID

Students can apply for disability-related accommodations with the Office of Accessibility Services (OAS) when they are accepted to attend FSU and have received an FSUID (e.g. abc26d). All communication will be conducted directly with the student through their FSU email address.

2. Submit an application for OAS Services

Complete the OAS Application. If you need a paper application or you would like to provide paper copies of your documentation, please contact OAS at OAS@fsu.edu or at (850) 644-9566.
  • Documentation is typically required to move to the next step of the OAS registration process. Documentation should:
    • indicate the specific diagnosis/es and the diagnostic criteria used.
    • describe the current and future impact of the disability.
    • include the name and credentials of the diagnosing provider with relevant expertise.
    • include the student's name.
  • The Healthcare Provider Documentation Form can be completed by a relevant licensed provider to fulfill documentation requirements.

3. Application Under Review

Each submitted application is assigned to an Accessibility Specialist for review.
  • Documentation is insufficient: the Accessibility Specialist will reach out to the student to request additional materials.
  • Documentation is sufficient: the Accessibility Specialist will reach out to schedule an intake meeting (in-person or via Zoom) with the student.

4. Meet with Your Accessibility Specialist

The interactive process necessary for determining appropriate accommodations will occur directly between the student and their Accessibility Specialist, but students are welcome to invite parents to attend. The student will share how their disability affects them academically and beyond and discuss any concerns they might have. Approved accommodations are reviewed at the end of the meeting.
  • If accommodations that were not discussed in the intake meeting are needed, the student can reach out to their Accessibility Specialist to discuss an Accommodation Review. Accommodation Reviews are follow-up meetings that continue the interactive process to ensure that disability-related needs are met.

5. Complete "Next Steps"

After the intake meeting, the Accessibility Specialist will set up the student's OAS profile and send an email with next steps and any resources discussed. All courses the student is enrolled in will populate in their OAS profile within two (2) business days.

6. Request Accommodation Letters for Each Class Every Semester

Every semester, students apply accommodations to their classes by generating Accommodation Letters. Letters are queued until approximately two weeks prior to the start of the term and continue to be distributed by request until the last (10) class days of the term.
